If you wish to be exempted from the class action and LIVE in Saskatchewan, you must return the forms mailed to retired employees of the provincial government by October 31, 2007. 

If you LIVE OUTSIDE of Saskatchewan and you wish to be included in the class action, you must return the forms by October 31, 2007. Witness to your signature can be anyone.
